Dolphin Dental Group Care to Share Referral Rewards Program Official Rules

1. No Purchase Necessary.

2. TO ENTER: Refer a new patient to Dolphin Dental Group and you will be entered in our grand prize drawing to be conducted every three months beginning 06/30/2012. Qualifying referral is defined as any patient age 18 or older who completes either a comprehensive examination or emergency care appointment. This patient must not have been seen in the practice previously.

3. PRIZE DETAILS: The winner will receive a Grand Prize to Be Determined for Each Drawing. Monthly and Quarterly Prizes must be claimed at Dolphin Dental Group: 10820 Seminole Blvd Seminole, FL 33778

4. DRAWING AND AWARDING OF PRIZE: Dolphin Dental Group will select one winner in a random drawing from all qualified entrants on June 30, 2012 and subsequently every three months. The drawing and awarding of prizes will be conducted by a representative of Dolphin Dental Group whose decisions on all matters relating to this drawing are final. The winner will be notified on the day of the drawing. The winner agrees that Dolphin Dental Group may use their names or likeness for advertising and publicity purposes without additional compensation (except where prohibited). Acceptance of the price indicates acceptance of all the contingencies described herein and inherent to the prize offered.

5. ODDS: Odds of winning depend on the number of qualified entries received. Minimum of three referrals in a one month period for Monthly Prize Qualification. For Quarterly Prizes, one entry per referral will be entered into drawing, no minimums or maximums.

6. ELIGIBILITY: Drawing open to legal U.S. residents age 18 or older. Entries are subject to verification by a representative of Dolphin Dental Group based on the following criteria: each referral entry must be associated with a unique treatment appointment for a patient not currently utilizing the Dolphin Dental Group practice. Employees of Dolphin Dental Group, its advertising and promotion agencies, and the immediate family and household members of each are not eligible. All federal, state, and local laws apply. Void where prohibited. Dolphin Dental Group and its advertising and promotion agencies, are not responsible for printing, distribution or production errors and may rescind, cancel or revoke this drawing based upon any such error without liability and at its sole discretion. By participating in this drawing, entrants waive all right to, and hold harmless, Dolphin Dental Group and its advertising and promotions agencies from any loss, damage or expense arising out of or in connection with participation in this drawing or acceptance, use or misuse of any prize.
